1. Our Commitment to Your Privacy

Agnee Funeral Services respects the privacy of the families and individuals we serve

We understand that arranging a funeral, cremation, burial or international repatriation may require families to provide detailed personal and family information at a difficult and emotional time.

Some of this information is required to arrange the funeral itself. Other information may be required to complete death registration, cremation, cemetery, government, statutory or other documentation on behalf of the family.

This Privacy Policy explains how Agnee Funeral Services collects, holds, uses and discloses personal information.

2. Information We May Collect

The information we collect depends upon the services being provided and the documentation required.

We may collect information including:

  • your name, address, telephone number and email address;
  • your relationship to the deceased;
  • information about the deceased;
  • information concerning the deceased’s spouse or partner, parents, children, siblings, next of kin and other family members;

  • information concerning marriages and relationships;
  • information required to complete death registration and other statutory documentation;
  • funeral, cremation and burial information;
  • information required to complete an application for cremation;
  • relevant medical practitioner and medical-device information where required for cremation;

  • other information reasonably necessary to arrange or provide the services requested.

We seek to collect only information reasonably required to provide our services, complete required documentation and meet our legal and administrative obligations.

3. Information About the Deceased and Their Family

Arranging a funeral and registering a death can require considerably more family information than people may initially expect.

Depending upon the circumstances, Agnee Funeral Services may need information concerning the deceased’s parents, spouse or partner, children, siblings and other family members.

This information may be required to accurately complete death registration, cremation, cemetery or other official documentation.

Information solely about a deceased person is generally treated differently from information about a living individual under Australian privacy law. However, information concerning a deceased person may also contain or reveal personal information about living family members.

Agnee Funeral Services therefore takes the privacy of family information seriously, regardless of whether it is supplied directly by the person concerned or by another family member arranging the funeral.

4. How We Collect Information

Agnee Funeral Services may collect information:

  • directly from you;
  • from another family member, next of kin, executor or authorised representative;
  • during funeral arrangements conducted in person;

  • through our website and Contact Us form;
  • through online forms provided using Jotform;
  • from hospitals, nursing homes, hospices and medical practitioners;

Where reasonably practicable, we collect personal information directly from the person concerned or from a person authorised or appropriately placed to provide that information.

5. Why We Collect and Use Information

Agnee Funeral Services collects and uses information where reasonably necessary to arrange funerals and provide related services.

This may include:

  • transferring and caring for a deceased person;
  • arranging a funeral, cremation or burial;
  • preparing and completing death registration documentation;
  • preparing cremation, cemetery and other statutory forms;
  • communicating with cemeteries and crematoriums;

  • administering and improving our services and business operations; and
  • meeting legal, statutory and regulatory requirements.

Some information is necessary because a government authority, cemetery, crematorium, medical practitioner or other organisation requires it.

If required information is not provided, we may be unable to complete particular documentation or provide a requested service.

6. Information About Other Family Members

A person arranging a funeral will often need to provide information concerning other members of the deceased’s family.

Where you provide personal information about another living person, we ask that you provide only information that is relevant and appropriate for the funeral arrangements, death registration or related documentation.

We recognise that it may not always be practical for Agnee Funeral Services to communicate directly with every family member whose information is required for official funeral or death-registration documentation.

Where required and reasonably practicable, we take steps to ensure individuals are made aware of how their information is handled.

7. Online Forms and Jotform

Agnee Funeral Services uses online forms to collect information for enquiries, funeral
arrangements, death-registration information, cremation documentation and related purposes.

Some of these forms are provided using Jotform, a third-party online form service.

Information submitted through Jotform is transmitted and processed using systems operated by Jotform and its service providers.

Agnee Funeral Services does not represent that information submitted through Jotform
is stored exclusively within Australia.

Information submitted through our online forms may therefore be processed or stored using infrastructure located outside Australia.

We use information submitted through these forms for the purpose for which it was collected and for related purposes reasonably necessary to provide the requested services or complete required documentation.
