The Death Certificate Data Collection Form.
Hi Family,
To enable me to submit the information required for the death certificate, please complete the following form.
Once the information has been submitted, the Registry will process the application. Please note that the certificate may take
approximately 5-8 weeks to be issued.
Steps to make your photo tribute presentation.
- Use a computer, Mac or laptop.
- Please do not use an iPad or mobile phone, as the output quality may be lower.
- Gather all photographs, videos and music you wish to use into a single folder on your computer or Mac.
- Download and use the desktop version of CapCut.
- Maximise each photograph to fill the screen so the full screen area is used.
- Export the completed presentation as an HD MP4 file.
- Use the form below to send me your file. If the upload does not work, you may use WeTransfer.
- The cut-off time for me to receive the file is 4.00 pm on the day before the funeral. This gives me time to verify the file and provide feedback if required.
- If you are unable to transfer the file electronically, save the video onto two USB drives and give them to me on the day of the funeral.
- Please test both USB drives beforehand and make sure the video plays correctly.
